  • Cancer cells can be destroyed more effectively and selectively with a unique new reusable treatment, activated with a substance found in stinging nettles and ants. Led by Professor Peter J. Sadler from Warwick s Department of Chemistry, researchers have developed a new line of attack against cancer: an organic-osmium compound, which is triggered using a non-toxic dose of sodium formate, a natural product found in many organisms. Named JPC11, it targets a metabolic process which cancer cells rely on to survive and multiply. It does this by converting a key substance used by cancer cells to provide the energy they need for rapid division(pyruvate) into an unnatural lactate leading to the cells destruction. Uniquely, this chemo-catalyst treatment can be recycled and reused within a cancer cell to attack it repeatedly. This unprecedented functional ability to recycle and reuse the compound within cancer cells could lead to future anticancer drugs being administered in smaller, more effective doses.   • Hearing impairment is currently treated surgically by replacing the bones with tiny prostheses, which leads to failure in as many as 25 to 50 percent of cases. To address this, researchers from the University of Maryland Medical Centre had employed 3D printing technology to manufacture custom-fitted ear bones(the smallest bones in the human body). The authors hope to run experiments with animal models and human cadavers in the foreseeable future. The main difficulties will be designing bones based on actual patients and finding biocompatible materials to minimise the risk of rejection. 3D printing is probably going to become a standard of care whenever there s a need for a prosthesis, whether it be a joint or a middle ear. The standard of care will not be an off-the-shelf component, but a component that s custom designed for that specific patient.

  • Happy February February is American Heart Month Every 40 seconds an American dies of cardiovascular disease. The risk of cardiovascular disease can be lowered by 80 through proper diet and exercise. That s why our role as health care providers is so important. Imagine how many people we can help and how much heart disease we can prevent through encouraging lifestyle and diet changes in our patients! I ve learned that patients are more likely to change their habits and be compliant when they understand how it will help them. For example: as a medical student I was seeing a patient recently diagnosed with high cholesterol. I explained to him the different types of cholesterol, their effect on the body if they are too high, and the foods responsible for raising each type. He thanked me for explaining this to him because no one had ever taken the time to educate him! He was excited to modify his diet and be more proactive about his health. All it takes is a few extra minutes but your impact can be powerful and long lasting.   • Speaking of& digestion Let s talk about anesthesia& pre-operative fasting. It s important for patients to fast prior to a scheduled surgery in order to decrease the risk of peri-operative pulmonary aspiration. A full stomach puts the patient at risk for aspirating gastric contents& can change your anesthetic management, increase their risk for aspiration pneumonia, respiratory distress, etc. Trauma patients are automatically considered to have full stomach and are managed as such. The recommendations for older children& adults presenting for elective surgery are as follows: Patients are recommended to fast from a fried or fatty meal for 8 or more hours before the time of their procedure. Patients should fast from a light meal for 6 or more hours prior to their procedure. Patients can have clear liquids(but NOT alcohol) up to 2 hours prior to their procedure.Not sure how many people would be drinking at 4am before their surgery, but hey you never know Make sure to confirm with your patients the last time they ate or drank something during your pre-op evaluation.   • Research and Science has motivated me to make a surprise phone call to a girl whose dad and brother are blind. I told her about my mission in trying my best to help the men in her house see for the first time. Yesterday, I visited them& after attaining their medical records, I contacted the company to check whether the electronic glasses would work for them. We all sat in the living room as I called, the phone was on speaker so everyone can hear. Unfortunately, after speaking to the agent, it was determined that the father s blindness level is so severe eliminating his chances to try on the electronic glasses. Sadly, I took a deep breath as I continued with the son s information, as soon as I finished saying his visual acquity is 20/200 We heard the agent say: Okay, good! He is eligible to try on the glasses and there s a chance he will see. I looked at the son who was sitting right next to me on the couch, all I saw was heavy tears running down his cheeks, it made me cry and gave me goosebumps! Suddenly, the entire family was crying. The blind father(65 years old), his blind son(26 years old) along with the mom and sister. Their tears lasted for minutes, it was such a scene filled with happiness, tears, and goosebumps! In the meantime, I will keep this family in my prayers hoping that our road trip on February 1st will be successful as we head to Ohio to try on the magical shades.   • I often get asked by medical student what resources they should be using in preparation for their anesthesia rotations Here are my list of resources based on personal experience& recommendations: Pocket Anesthesia(photographed above) The layout is similar to the rest of the pocket series books. My resource. Really great, high yield facts, to the point. Even as a resident this book is really great to have on hand. I m glad I made this purchase as a medical student. And like it says, fits right in your pocket. I pulled this bad boy out when I had downtime in the OR. Great to have on hand& lots of questions can be sparked while reading this in the OR, giving you and your preceptor more topics to talk about. Stanford Guide to CA-1. This is a FREE resource available online as a PDF. Meant to prepare incoming anesthesia residents for the OR. Serves as a great introduction to basic anesthesia. Perfect for medical students& did someone say FREE! It s around 75 pages of PowerPoints and an easy read. This is a MUST have for medical students rotating in anesthesia. Baby Miller s Basics of Anesthesia. This book serves as an introduction to anesthesia and covers all of the basics. It s easy to understand& doesn t go into extensive detail but just enough for someone who is starting out and creating a foundation for their anethesia knowledge. Keep in mind many residency programs will provide this for you once you start residency. So you may want to hold off, borrow a copy, or buy an older edition until then. Not a necessity but a great resource to reference after work or prior to a particular procedure when you want a little more clarification on anesthesia topics. Duke s Anesthesia Secrets. I haven t personally used this resource but a few of my old classmates brought this one to my attention. The layout looks similar to the other Secrets series books.   • TRUE STORY: I was on my last month of rotations as a 4th year medical student. One morning as I was standing in line at the coffee cart to grab my daily dose of caffeine, a little girl came up beside me. Noticing my white coat, she excitedly turned to her father and said look daddy, she s a doctor. Nurse*, one of the guys that I was rotating with quickly corrected her and smirked at me. I turned to him and gave him a look of disapproval. His words didn t hurt me, but I could see the look of disappointment on the little girls face when she discovered she had incorrectly assumed I was a physician. Although my friend was only kidding, his joke further perpetuated the stereotype that a woman s role in medicine was not that of a physician. Even now as a resident many patients ask me what my role is in their care, despite me introducing myself as their doctor. I m both inspired& motivated to break down these stereotypes, alongside the many amazing and brilliant women in medicine This post is inspired by story.   • Happy Anesthesiologist Week Did you know that the first successful public demonstration of anethesia was actually performed by a Dentist! Prior to this, surgeries were performed using techniques such as vigorous carotid massage until the patient was rendered unconscious, giving patients alcohol, opium, and mixtures of herbals to minimize pain. Everything changed on the crisp Fall morning of October 16th 1846, in the city of Boston, at what is now known as the Ether Dome inside Massachusetts General Hospital. Dr. William T. G. Morton proved to the world that the use of ether could provide anesthesia for the excision of a neck mass. It wasn t until 1956 that the modern-day brominated general anesthetic Halothane was introduced by Dr. Michael Johnstone. Since then the field of anesthesia has remarkably progressed and the administration of anesthesia has become exceptionally safer. I m thankful to be a part of the modern era of anesthesia, but am always in awe of the curiosity which lead to the discovery of anesthetics.

  • Applying to residency as a DO: Why I took the USMLE& COMLEX It s that time of year when 2nd year medical students across the nation are preparing& studying diligently for their looming board exams. As a DO student, it can come with the extra stress of taking not only one board exam, but two! The COMLEX must be taken for licensing& graduation requirements but USMLE is optional. Although, I really didn t see it as an option, but instead a requirement for applying in to anesthesia. I knew early on I wanted to apply to ACGME programs. Sure, some ACGME programs may accept the COMLEX, do they really understand how the test is scored, and what numbers are considered competitive I recall seeing programs requiring a minimum USMLE score of 210(20 points below ave) or a COMLEX of 600(50 points above ave) just to qualify for a rotation. Those scores are not comparable at all lol. I decided that I was going to take both exams in order to give myself the best possible shot at matching into a competitive program. Why close any doors on myself I ended up taking both Step 1 and Step 2 prior to applying for the match. Some anesthesia programs won t send out an interview invite unless they have your Step 2 score on file. I urge those planning to apply to anesthesia, or any other competitive field, to take the USMLE. I personally don t know anyone who matched in to anesthesia with just a COMLEX score, so I don t have any experience to share with you if that is your plan unfortunately. However, I have spoken to upperclassmen applying in to other fields who regretted not having taken the USMLE. Good luck to all you 2nd years out there!.

  • Happy FriYAY everyone I wanted to discuss a VERY important topic that I think is brushed under the rug a lot of times, MENTAL HEALTH! Starting residency was a very big change: new city, new home, new friends, new environment, new staff, new computer systems, huge responsibilities, very sick patients, the list goes on. It was honestly a lot to adjust to,& extremely stress provoking. Looking around me, other interns seemed comfortable in their new role, which made me feel like something was wrong with me. I felt alone, residency can be very isolating at times. In Med school you have friends, you have an identity, you have time to spend with loved ones. Residency changes all of that, you are forced to make new friends with the little free time you have, you lose the identity you had for 4 years, and you lose touch with family& friends. I started to really miss being a Med student and all of my old friends. After months of stress and anxiety I finally reached out to a professional who recommended the book The Happiness Trap. This book was a GAME CHANGER for me. I cannot even begin to explain how much the techniques in the book helped me get through my extremely tough time. I also purchased Mind Over Mood after seeing recommend it. As I became more comfortable, I began expressing how I felt to my co residents,& to my surprise they were all experiencing the same feelings and concerns! I came to the realization that as health care professionals we are SO good at putting on a front that we are okay even if deep down we are fighting turbulent emotions. We hold ourselves to such high standards. WE take care of the sick, WE are not the sick. It s difficult to accept that sometimes, as hard as you try not to, you are bound to feel anxious, depressed, panic, sadness, etc in life. Just remember, YOU are not alone! If you re having an internal battle and struggling please REACH OUT, start opening up to your peers, mentors, counselors. I promise you the hardest part is reaching out, but once you do, you ll wonder why you let those feelings linger for so long. Please don t neglect your own health& well being have a fabulous weekend

  • Life is extra* sweet when you re on OB& new babies are being born Let s talk about regional anesthesia in pregnancy. Pregnant patients can be a challenge because so many physiological changes occur in a woman s body during pregnancy. Her total blood volume 25-40, plasma volume 40-50, in cardiac output, in O2 consumption, in fibrinogen, along with other changes. The most commonly used regional techniques in labor& delivery include a spinal, epidural, or combined spinal epidural block. The most common complication of these blocks is hypotension due to sympathectomy. For this reason, it s important to assess the volume status of your patient by monitoring their blood pressure closely prior to giving a neuraxial block. Let s talk about the differences between an epidural& a spinal block. Epidural: Injection of med is done in the epi(above) dural space between the ligamentum flavum and the dura mater. requires a higher dose of anesthetic can be done at any level of the vertebral column onset is slower than a spinal you achieve a less dense block so it is less potent than a spinal in terms of neuromuscular blockade. can be dosed multiple times using a catheter Spinal: Injection of med is in the subarachnoid space requires a lower dose of anesthetic must be given below the termination of the spinal cord(L1/2) typically around L4 onset is quicker and the block is more dense pt can only be dosed once. Aside from the baby, the anesthesiologist is probably the 2nd favorite person in the room.

  • Taking this photo this morning helped me reflect on how amazing the human body is. In order to see this image, light was processed by the eye, which contains around 95 million cells. Our brain doesn t work like a camera, instead it constructs a model of the world from information about light and shade, edges, curvature etc. We have a blind spot in each eye, an area at the back which doesn t have any sensory cells. So the brain uses all the information it has to compensate so we aren t even aware of it! It s common to feel disappointed by our bodies.
