  • What happened recently in Las Vegas was nothing short of a tragedy. My thoughts and prayer are with all the families and friends of all the fallen. The saddest I felt was the reports of Sonny Melton, a 29 year old nurse who was at the festival with his wife. He grabbed his wife and started running, effectively shielding her from an attack. His wife felt the moment he was shot in the back, a blow that would latter take his life. Sonny is an example of protecting others even to your final heartbeat, that s why he was a fine nurse, his time and future were cut far to short. There were many great men and women who s lives ended far to soon, I want to remember them all. Thank you Sonny for your service to the health care community, you shall not be forgotten in my lifetime.

  • Spondylolisthesis is a pathology of the vertebral column characterized by the sliding of one vertebra on the other. The symptoms and problems typical of spondylolisthesis depend on the degree of sliding and the speed with which this occurs. Generally it manifests itself with a slight low back pain characterized by a slight but annoying pain in the area where the vertebral sliding occurred. The pain is more localized in the positions that accentuate the lumbar lordosis(extensions of the spine) and is reduced in the positions that decrease it(flexion of the spine). The Bird Dog is the most basic of basic exercises to building lower back and posterior chain stability. If you re currently trying to rehab your bad back and your not doing these I hope and pray you have a darn good reason. In todays post I want to make sure you walk away with two important How To s when it comes to the Bird Dog.  The Bird Dog is an exercise that looks super easy but can be jacked up in so many ways. If you are the average pain free person sure, you can swing your arms and legs a couple times and pull it off with no problem but for us pain riddled individuals, we need to be super careful how we execute this move. Before you start doing anything prescribed on this post it s really important to lean what your pain triggers are. I can t stress that enough. If you really don t know what is making your pain worse then you really won t get anywhere with this exercise.

  • Woke up ate and wanted to go back to sleep. But then I stared convincing my self that I can do this without hurting my self any further. I did a hit leg today for about 1 hours 45 mins. All light weights and use of machines to take pressure away from my stomach and not making it any worst. Here s a few workout from today s secession. Leg Day- Forward Box Squat(substituted with bench because boxes where occupied)- 5 sets of 20 Reps blasting my quad and hammies.- Regular/Reverse Box Squat(substituted with bench)- 5 sets of 20 reps continuing blasting my quads and also clenching the glutes when up.- Smith Machine Split Lounges- 5 sets of 10 reps per leg.- 40 Degree Hack Squat- 5 sets of 10- 12 Reps- Forward Power Squat- 5 sets of 10 reps. I tried to go really low but couldn t at this point my body starting to feel really irritated.- Reverse Power Squat- 5 sets of 10 reps.   • I ve been trying to think of ways to add value to this page, and decided that video tutorials might be a good way to do that. Here s a video of me saving a banana s life with one of the most commonly used sutures the simple interrupted suture. One of the most important things to keep in mind when suturing is to avoid applying excessive force when approximating the tissues(i.e. don t tear through your patient s gingiva when throwing your suture). Being able to suture a banana without pulling the thread through the peel is a good way to practice. DM or comment with any questions/criticisms/suggestions for improvement. Hope you enjoy!

  • Happy February February is American Heart Month Every 40 seconds an American dies of cardiovascular disease. I ve learned that patients are more likely to change their habits and be compliant when they understand how it will help them. For example: as a medical student I was seeing a patient recently diagnosed with high cholesterol. I explained to him the different types of cholesterol, their effect on the body if they are too high, and the foods responsible for raising each type. He thanked me for explaining this to him because no one had ever taken the time to educate him! He was excited to modify his diet and be more proactive about his health. All it takes is a few extra minutes but your impact can be powerful and long lasting.   • Speaking of& digestion Let s talk about anesthesia& pre-operative fasting. It s important for patients to fast prior to a scheduled surgery in order to decrease the risk of peri-operative pulmonary aspiration. A full stomach puts the patient at risk for aspirating gastric contents& can change your anesthetic management, increase their risk for aspiration pneumonia, respiratory distress, etc. Trauma patients are automatically considered to have full stomach and are managed as such. The recommendations for older children& adults presenting for elective surgery are as follows: Patients are recommended to fast from a fried or fatty meal for 8 or more hours before the time of their procedure. Patients should fast from a light meal for 6 or more hours prior to their procedure.   • Does Early Range of Motion Lead to More Rotator Cuff Repair Failures- by-⠀ WANT TO LEARN MORE FROM ME Head to my website MikeReinold.com, link in bio.⠀-⠀ If you work with any rotator cuff repair patients, you ve probably noticed that the surgeons are starting to delay rehab and range of motion more and more. Several years ago we started to learn that the failure rate of rotator cuff repair procedures was higher than expected. And the larger the tear, the higher the failure rate. ⠀.⠀ However, it really depends on how you define failure. The surgeons define it as a retear. However, most patients, even those that failed, had higher subjective outcomes. Regardless, the docs blamed early rehab and started to slow it down.⠀.⠀ A recent systematic review out of Rush from JSES showed that there was no clear superiority noted in clinical outcome scores for early-motion or delayed-motion rehabilitation. Results of tendon healing were found to be either no different or in favor of delayed motion, but no differences were noted in rotator cuff tear recurrence rates postoperatively. The majority of meta-analyses found significantly better range of motion with early motion up to a year postoperatively for forward elevation and up to 6 months for external rotation, but significant differences were not reported for functional improvements and strength at 12 months postoperatively. Subgroup analyses suggested that larger preoperative tear sizes have significantly greater retear rates with early-motion rehabilitation. ⠀.⠀ Based on this, I m still doing early ROM for most people. I ve been doing it for 20 years. My resource. Really great, high yield facts, to the point. Even as a resident this book is really great to have on hand. I m glad I made this purchase as a medical student. And like it says, fits right in your pocket. I pulled this bad boy out when I had downtime in the OR. Great to have on hand& lots of questions can be sparked while reading this in the OR, giving you and your preceptor more topics to talk about. Stanford Guide to CA-1. This is a FREE resource available online as a PDF. Meant to prepare incoming anesthesia residents for the OR. Serves as a great introduction to basic anesthesia. Perfect for medical students& did someone say FREE! It s around 75 pages of PowerPoints and an easy read. This is a MUST have for medical students rotating in anesthesia. Baby Miller s Basics of Anesthesia. This book serves as an introduction to anesthesia and covers all of the basics. It s easy to understand& doesn t go into extensive detail but just enough for someone who is starting out and creating a foundation for their anethesia knowledge. Keep in mind many residency programs will provide this for you once you start residency. So you may want to hold off, borrow a copy, or buy an older edition until then. Not a necessity but a great resource to reference after work or prior to a particular procedure when you want a little more clarification on anesthesia topics. Duke s Anesthesia Secrets. I haven t personally used this resource but a few of my old classmates brought this one to my attention. The layout looks similar to the other Secrets series books.   • TRUE STORY: I was on my last month of rotations as a 4th year medical student. One morning as I was standing in line at the coffee cart to grab my daily dose of caffeine, a little girl came up beside me. Noticing my white coat, she excitedly turned to her father and said look daddy, she s a doctor. Nurse*, one of the guys that I was rotating with quickly corrected her and smirked at me. I turned to him and gave him a look of disapproval. His words didn t hurt me, but I could see the look of disappointment on the little girls face when she discovered she had incorrectly assumed I was a physician. Although my friend was only kidding, his joke further perpetuated the stereotype that a woman s role in medicine was not that of a physician. Even now as a resident many patients ask me what my role is in their care, despite me introducing myself as their doctor. I m both inspired& motivated to break down these stereotypes, alongside the many amazing and brilliant women in medicine This post is inspired by story.   • Happy Anesthesiologist Week Did you know that the first successful public demonstration of anethesia was actually performed by a Dentist! Prior to this, surgeries were performed using techniques such as vigorous carotid massage until the patient was rendered unconscious, giving patients alcohol, opium, and mixtures of herbals to minimize pain. Everything changed on the crisp Fall morning of October 16th 1846, in the city of Boston, at what is now known as the Ether Dome inside Massachusetts General Hospital. Dr. William T. G. Morton proved to the world that the use of ether could provide anesthesia for the excision of a neck mass. It wasn t until 1956 that the modern-day brominated general anesthetic Halothane was introduced by Dr. Michael Johnstone. Since then the field of anesthesia has remarkably progressed and the administration of anesthesia has become exceptionally safer. I m thankful to be a part of the modern era of anesthesia, but am always in awe of the curiosity which lead to the discovery of anesthetics.

  • Applying to residency as a DO: Why I took the USMLE& COMLEX It s that time of year when 2nd year medical students across the nation are preparing& studying diligently for their looming board exams. As a DO student, it can come with the extra stress of taking not only one board exam, but two! The COMLEX must be taken for licensing& graduation requirements but USMLE is optional. Although, I really didn t see it as an option, but instead a requirement for applying in to anesthesia. I knew early on I wanted to apply to ACGME programs. Sure, some ACGME programs may accept the COMLEX, do they really understand how the test is scored, and what numbers are considered competitive I recall seeing programs requiring a minimum USMLE score of 210(20 points below ave) or a COMLEX of 600(50 points above ave) just to qualify for a rotation. Those scores are not comparable at all lol. I decided that I was going to take both exams in order to give myself the best possible shot at matching into a competitive program. Why close any doors on myself I ended up taking both Step 1 and Step 2 prior to applying for the match. Some anesthesia programs won t send out an interview invite unless they have your Step 2 score on file. I urge those planning to apply to anesthesia, or any other competitive field, to take the USMLE. I personally don t know anyone who matched in to anesthesia with just a COMLEX score, so I don t have any experience to share with you if that is your plan unfortunately. However, I have spoken to upperclassmen applying in to other fields who regretted not having taken the USMLE. Good luck to all you 2nd years out there!.

  • We are bound by our bridges, We are bound by our stars. In a world filled with oceans, filled with trouble, filled with scars. I want to take a moment to apologize for my prolonged social media absence. Lately, I have felt much like the ice of Diamond Beach- relentlessly battered and tossed about by life with no chance for escape or peace. As you all know, I had knee surgery in November and have spent the past several months recovering. During this time, I ended up medically withdrawing and taking a medical leave from work. Financially, this put me in a stressful situation and was a very difficult decision for my education and career. The worst moment to come was New Year s Eve when I slipped on ice outside of my home and ended up in the Emergency Room unable to walk. I am back on my feet but since my fall I have experienced recurring knee pain that was not previously a part of my recovery and is a huge concern. I will not know for another month if I need a second surgery because I cannot afford a second MRI. I am struggling to stay afloat, but the waves are merciless in their attack and I can t seem to catch a break. I am exhausted and in immense pain, but am trying to keep in mind that this storm will pass and I will make it out of this with a strength I do not yet possess. My scars will be a testament to all that I have overcome, and believe you me, I will overcome this. The ice on Diamond Beach is exquisite and beautiful as a result of constant assault by the water, and so too will this part of my life be beautiful and defining when I reflect back one day. Just not today. Thank you all for remaining a part of my journey, and I hope to be in a better place soon.

  • I am 2 weeks post knee surgery now and can slowly walk without the crazy pain from beginning. Time to get back on track and focus on some serious chest workout. I still have a LOT recovery to do for my knee and legs but nontheless my upper body strength is on a level that I only achieved once in life- 2 years ago. I am excited how much I m able to do with always keeping up my positivity when life hits you down.   • I love you- every blemish tells a special story! Look at your life because it can change so quickly. Enjoy the little things even if they can sometimes be annoying. It means so much when you can go to work, stand on your own legs, or eat what you want. But also things like cleaning the apartment or going shopping- we often take these for granted. brings up such a good point about when we re recovering and not able to even do the mundane things that are ughhh chores- we can miss that! The freedom to do what we want when we want can be taken away so quickly. Perhaps when you re doing a chore today, consider how maybe there was a time when you couldn t physically do this and how you wished you could have been able to. Nothin like a lil ol perspective shift to brighten your mood.
